1. Introduction

This Telehealth Consent explains what telehealth is, its benefits and limitations, and asks for your informed consent to receive care through Auva RX using telehealth technology. Please read this carefully before beginning any consultation.

2. What Is Telehealth

Telehealth is the use of electronic communications (such as online questionnaires, secure messaging, phone, or video) to enable a licensed healthcare provider to evaluate, diagnose, treat, and prescribe for a patient without an in-person visit. Providers you interact with through Auva RX are independently licensed and are solely responsible for all medical decisions, including whether treatment is appropriate for you.

3. Benefits of Telehealth

Telehealth may allow you to receive care conveniently from home, avoid travel and wait times, and access licensed providers more quickly than a traditional in-person visit.

4. Risks and Limitations of Telehealth

As with any medical care, telehealth has risks and limitations, including but not limited to:

  • Information transmitted electronically may, in rare instances, be intercepted, lost, or accessed by unauthorized parties despite security safeguards.
  • The absence of an in-person physical examination may limit a provider's ability to fully assess your condition.
  • Technical difficulties may delay or interrupt your consultation.
  • Telehealth is not appropriate for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately.
  • Your provider may determine, based on your intake information, that telehealth is not appropriate for your situation and may decline to prescribe treatment or may refer you to in-person care.

5. Your Responsibilities

You agree to provide accurate, complete, and truthful information about your health history, current medications, symptoms, and any other information requested during your consultation. Incomplete or inaccurate information may affect your provider's ability to safely evaluate and treat you.

6. No Guarantee of Treatment

Completing an intake assessment does not guarantee that you will be prescribed medication. All prescribing decisions are made solely by your independent, licensed healthcare provider based on their clinical judgment.

7. Compounded Medications

If prescribed, certain medications offered through Auva RX may be compounded medications prepared by a licensed pharmacy based on your individual prescription. Compounded medications are not reviewed or approved by the FDA for safety, effectiveness, or quality.

8. Consent

By proceeding with a consultation through Auva RX, you acknowledge that you have read and understood this Telehealth Consent, understand the risks and limitations described above, and voluntarily consent to receive care via telehealth.
