This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
1. Our Commitment to Your Privacy
Auva RX and the licensed medical providers who evaluate your health assessment are committed to protecting your Protected Health Information ("PHI") in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
2. How We May Use and Disclose Your PHI
We may use and disclose your PHI for the following purposes:
- Treatment — to facilitate evaluation and care by licensed medical providers, and to coordinate prescription fulfillment through our pharmacy partner (Rush Pharmacy) when treatment is prescribed.
- Payment — to process payment for services rendered.
- Healthcare Operations — to support quality assurance, provider oversight, and administrative functions necessary to operate our Services.
- As Required by Law — when disclosure is required by federal, state, or local law.
We will not use or disclose your PHI for any other purpose without your written authorization, except as described in this Notice or as otherwise permitted or required by law.
3. Your Rights Regarding Your PHI
You have the right to:
- Request access to your PHI.
- Request a correction to your PHI if you believe it is inaccurate or incomplete.
- Request a list of certain disclosures of your PHI.
- Request restrictions on certain uses and disclosures of your PHI.
- Request confidential communications in a manner or at a location you specify.
- Obtain a paper copy of this Notice upon request.

4. Our Responsibilities
We are required by law to:
- Maintain the privacy and security of your PHI.
- Provide you with this Notice describing our legal duties and privacy practices with respect to your PHI.
- Notify you if a breach occurs that may have compromised the privacy or security of your PHI.
- Abide by the terms of this Notice currently in effect.
5. Changes to This Notice
We reserve the right to change the terms of this Notice and to make the new Notice provisions effective for all PHI we maintain. Any revised Notice will be posted on our website.
6. Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at support@auvarx.com or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
